OUR GOAL IS TO MEET INDIVIDUAL NEEDS BY PROVIDING QUALITY PERSONAL CARE AND EMPOWER OUR CONSUMERS TO LIVE INDEPENDENTLY IN THE COMMUNITY.
UHCC provides quality care for Seniors, Mentally ill, Developmentally Disabled, Autism and those recuperating from illness or injury. Our Care-givers make a difference in clients' lives and the lives of loved ones'. Care-givers work independently with our clients each day. We offer flexible schedules, including days, evenings, overnights and weekend hours.
BEHAVIORAL HEALTH SERVICES UNDER DCF:
Intensive In-Community and Behavioral Assistance for Children and their family.
We provide In-Community Services to Children and Family with specific challenges: Mentor, Tutoring, In-home Counseling/Therapy, Crisis Intervention and Stabilization, Behavioral modification. We come into the home and provide training to the Caregiver in Budgeting & Maintaining the home.
Children & Youth Summer Camp Program for the Special Needs:
Summertime is all about fun. But with that fun, there are also lots of opportunities for to learn new skills, such as building leadership skills, independence, and self-esteem. UHCC summer camp will concentrate more on having fun, and staying active with learning games and physical activities of all levels.
Every group is different, and every day is different as well. Good counselors have a bag tricks for the various personalities, disabilities or weather conditions that they come across.
Focusing on leadrship skills that will broaden their horizon and gain skills that will be useful in thier daily lives.
Supported Employment is provide to assist persons in obtaining and maintaining competitive employment in the community and encourages work, social interaction, and integration. Staff is assigned to the work environment.
Respite care is provided in the Consumers’ home, in an approved home or in the Community to temporarily relief the primary care-givers of their responsibilities and could be in a form of extended weekend care.
Under DCF we provide After-School, Weekend & Agency Respite Services
Hotel Respite basically has the same definition as Respite except it is provided in a "hotel" setting for individuals still living at home that needs supervision.
Our Children and Adult Respite Weekends now offer everybody the opportunity to get away from it all by taking a relaxing weekend in the New Jersey shores. We start with dinner in a restaurant on Friday evening to kick off the weekend. Saturday filled with activities ranging from breakfast on the boat during boat rides, visiting historic sites, outdoor fun, fishing, strolling the famous Boardwalks with carnival like atmosphere—dress up for formal dinner at a restaurant.

#3: Tomahawk Lake:
Summer Camp at Tomahawk Lake is spectacular & entertaining for young adults and children alike. The parks grounds come complete with everything a family could ask including miniature golf course, Softball field, horse-shoe and volleyball areas, an old fashioned picnic area and white sand beach featuring an incredible lake, parking, beach, swimming facilities, Papoose Land, Kiddie Water World, Cherokee Corner, water slide rides, swan boats, duck boats, paddle boats and bumper boats. Children will be swept away by all the activities offered.
Individual Support is provided in the Consumers home or in the community in the area of daily living skills and personal assistance: The Personal care provider assist/supervise in accordance with individual specific approved plan of care as provided to the agency: Daily living skills include basic household task, grooming, budgeting, meal planning, shopping, appointments and other errands essential to community integration.
Housing is provided for individual consumer who needed to mainstream into the community. Consumers are assisted with housing needs through Licensed Real Estate Brokers working with the agency.
Independent Living Program: we work with the individual consumer and the family members to develop (a Transitional) Independent Living Plan focused on a successful transition to the community or living independently from the family. We work with consumer through empowering on the skills needed to be independent.
Neurological, Brain and those recovering from illness or injury from accidents: Our services are designed to help these individuals to live more productive, independent lives.
Skills focused on are:
- Job attainment and maintenance - Vocational training
- Locating and maintaining housing - Banking and budgeting
- Awareness of community resources - Socialization
- Drivers' training and education - College preparation.
- Career exploration - Nutrition and Health
Program for Young Adults or Adults
Transition from Group Homes or from an Institution to the Community.
Assistance in Community Activities.
Participating and utilizing community Resources; Library, Gym, Theater, Hospital, Bank, School, Malls, Supportive living.
Other errands that will support and foster independency.

In-home academic tutor:Tutoring is available in all core subjects areas. Reading, mathematics, science, history etc.,This program is created with one simple objective: To offer highly effective one on one academic instruction in the convenience of your own home or a private setting such as a local library.
Services for Senior:
UHCC provides quality care for seniors. Our staff are trained to assist elderly and disabled persons with the tasks of dailiy living, to allow them to continue living independently in the comfort of their own homes. Our Caregivers make a difference in their lives and the lives of others.

Unique Home Care offers the following services on short term and long term basis:
Individual Support is provided in the Consumers home or in the community in the area of daily living skills and personal assistance: The Personal care provider assist/supervise in accordance with individual specific approved plan of care as provided to the agency: Daily living skills include basic household task, grooming, budgeting, meal planing, shopping, appointments and other errands essential to community integration.
Habilitation is provided in the Consumers’ hone or in the community in the areas of social, health, welfare, communication, by empowering Consumers through the provision of individual desire for independency as per plan of care. Included are: arranging use of community resources, recreational outing, school, training in money management, work, handling of emergencies, housing, recognizing street and other signs, voluntarily involvement in a socialization or self-help groups, develop self confidence when interacting with others, appropriate assertiveness, the use of public or private transportation, encourage in community participation such as volunteer, training or like interest.
Companionship and Social interaction are among the most important services we provide. We also perform other non-medical services such as meal preparation, light housekeeping, shopping, errands and transportation to doctors or therapy sessions.
Most importantly we help Seniors, the Mentally Challenged and those recuperating from illness or injury maintain their independence.
